Selecting the Best Fit FIM Program

Program Considerations

The table below provides high-level considerations under three key FIM interventions: MTM, MTG, PRx. These programs may be additionally supported by medical nutrition therapy (MNT) and culinary medicine (CM) programs where available and appropriate.

Complementary Education and Behavioral Support Program Considerations

Patients typically require ongoing nutritional support before, during, and after participation in FIM programs. Here, we highlight two such programs, though additional health system, digital, and community-based support programs may be available to patients.

Medical Nutrition Therapy

MNT may be beneficial for patients who need an individualized, evidence-based treatment plan and counseling from a registered dietitian nutritionist (RDN), such as those managing uncontrolled diabetes or multiple co-morbidities.

Culinary Medicine

CM may be well suited for patients who demonstrate high engagement in their care process and have already received or are actively receiving individualized dietary counseling. CM programs provide patients with additional food preparation and culinary skill training and peer support.

These nutrition education resources can provide additional guidance for patients as they participate in FIM and complementary support programs.
